Business Name or DBA
Tax ID
Copy of Business License

Upon receiving these documents, we will review your application and verify the provided documents. We will then create a Retailer account that you can use to log in to this website and take advantage of our trade discounts. We will let you know whether you have been approved, denied, or if further verification is needed. LA Furniture Store reserves the right to refuse service to anyone.
